  • INSTRUCTIONS:

    Complete Section A.  This will prompt an email to be sent to the instructor of the class requesting additional information.

    You will be required to submit a written statement with this form.  Your statement and your instructor’s feedback will be submitted to the Committee on Academic Standing for further review and decision. 

    You will be notified, via your @albany.edu e-mail, of the decision of the Committee on Academic Standing. Please check your e-mail frequently for correspondence. 

    You must submit one form per course.

    False statements and/or filing forged documents and/or submitting false material to a University committee violates the standards of academic integrity. Such violations are subject to appropriate disciplinary action.

  • Please upload a written explanation below of why you were unable to submit your request by the published deadline. If applicable, upload supporting documentation or other substantiating evidence. The following are not sufficient reasons for a policy exception:

     

    •  being unaware of the policy or deadline
    • you did not receive course evaluations/assessments/grades from the instructor prior to requesting a change from A-E to S/U or from S/U back to A-E
    • changing your mind because you performed better or worse than expected in a class
    • in order to demonstrate satisfactory academic progress or to avoid academic actions
    • to reintroduce GPA impacting grades  
    • to meet institutional GPA requirement for graduation
    • to adjust your GPA according to transfer plans
    • to adjust your GPA to remain eligible for financial aid

    Note: If you have already received your bachelor's degree from UA you do NOT qualify for this exception.

    Courses above 300 level cannot be S/U unless they have been designed by departments or schools as S/U graded. As of the fall 2018 semester, you cannot initiate a request for this exception that dates back more than one academic year.

  • Once you submit Section A of this form, your instructor will receive an email to complete Section B. Please check with your instructor to make sure Section B gets completed. In order for this request to be reviewed, both sections must be completed.
