2. Locate your SFS ID Number
- You will need the 10-digit SFS ID Number for yourself or your high school, depending on who the stipend will be issued to.
- If you do not know your SFS ID, contact the Statewide Financial System at 855-233-8363.
3. Verify your Address on File with the SFS
- Log into your SFS account to confirm the address on file is correct. If you have any trouble, contact the SFS directly.
- If the address is incorrect, request an official address change through their site.
- Do not submit this form until the updated address has been fully processed and confirmed (this may take some time).

Stipends will be mailed only to the address on file with the SFS. This address must match the address you list on this form.
It is the teacher's sole responsibility to ensure all SFS information is correct before submitting.
Once a stipend is processed, it may not be reissued or corrected.
