    Please follow the instructions precisely.

    1. Check with your high school principal (before submission of this form) regarding any restrictions that might apply to receiving personal stipend compensation. Your confirmation below denotes that you have received the appropriate approval from your school district to have the stipend issued to you or to your school as indicated on this form.

    2. Know your personal SFS ID Number (or the high school SFS ID Number if the stipend payment is being paid to your high school).

    a. If you do not know your 10-digit SFS ID, contact the Statewide Financial System at 855-233 8363.

    3. Confirm your address on file with SFS at: http://www.sfs.ny.gov/. Call the SFS if you run into any issues with their system.

    a. If the address they have is incorrect, request an address change.

    b. Confirm that the address has been changed (could be a few weeks). DO NOT SUBMIT THIS FORM UNTIL THE ADDRESS IS CONFIRMED CORRECT.

    Please note: Stipends will be mailed to the address on file with the SFS, which must be the same address listed on this form. It is the sole responsibility of the teacher to verify that the address on file with the SFS is correct. Once the stipend is processed, it cannot be revised and a new stipend will not be sent. Teachers must verify the address with the SFS before submitting this form to UHS.

    4. Type the information necessary on this form and confirm where designated.
