This release of student disciplinary records request form offers the following options for Current Students:
- Provide the Office of Community Standards permission to discuss your conduct record with a parent, third party or your advisor for the conduct process
- Request a copy of your disciplinary records be sent to your self or a third party.
- Revoke your permission to give a third party acces to your records

Pursuant to SUNY’s Records Retention and Disposition policy, all student disciplinary records will be retained for a period of seven (7) years from the end of the academic year in which the Student Conduct Referral was resolved. However, there is one exception, student conduct referrals resulting in either disciplinary dismissal/expulsion or a transcript notation of “withdrew with conduct charges pending” will be kept indefinitely.

As per the Records Retention Policy, if Community Standards has a student disciplinary record, they may only release the record in accordance with the Family Education Rights and Privacy Act (FERPA). As such, students must authorize the University to release their disciplinary records. We encourage you to direct the student to complete this form themselves to approve your access to their disciplinary records.
