    Each student may apply for one degree per submission of this form. If applying for multiple degrees/certificates, you must complete this form once per eligible degree. 
  • Active students must apply to graduate for the current term through the Registrar's website. 

  • This form cannot be used to apply to graduate for a future term. 

    In order to see when future graduate applications will be available, please visit the Academic Calendar.

  • Only one diploma is awarded to each undergraduate student indicating a Bachelor of Arts or a Bachelor of Science.

    Students may complete more than one major, designating which is to be considered the “first major, the “second major”, etc. The “first major” designates the type of degree awarded (Bachelor of Arts or a Bachelor of Science). If you need to make any changes to your majors, minors, concentrations, etc., please complete the Major Plan Change Form or Minor Plan Change Form

  • Please visit our website for information about updating your name and address to ensure your diploma is printed and shipped correctly. If you do not remember your UAlbany Net ID and password, you will need to Contact ITS to have this reset so that you can log in.

    By signing your name (typing your legal name) below, you acknowledge and certify that all information is correct, you are the student submitting this form, and authorize our office to submit your graduation application and review you for the above listed term. 

    You will receive an email confirmation of your request after hitting the submit button. The review for prior term graduation applications can take 7-10 business days. You will be notified once your review is complete.
