Honors Medallion Request

Please fill out this form if your fall grades did not allow you to graduate with honors, and you became eligible once spring grades were posted or if you applied to graduation late. Once the form is processed by the Office of the Vice Provost and Dean for Undergraduate Education, you will receive a notification. Should your application be approved, it will allow you to purchase an Honors Medallion from the university bookstore.
